Master IEP & 504 Accommodation Tracker
Built for special education teachers and case managers carrying a whole roster of plans. One matrix holds every student's accommodations, settings, service providers, and review dates — and the weekly log records what was actually delivered, period by period, including the refusals most trackers never capture.
What's inside
- Page 1 — Student overview & accommodation matrix. Every student, every accommodation, the setting, the service provider, and the expiration/review date on a single page you update whenever a plan changes.
- Page 2 — 25-item accommodation bank. Presentation, timing & scheduling, setting, and response supports, pre-written in plain language — check the items in the student's plan instead of wording them from scratch.
- Page 3 — Weekly execution log. Checkbox-first grid by period and day: P (Provided), R (Refused — the student declined), or N-A. Undated, so a missed week can be backfilled without breaking the record.
- Page 4 — Communication audit & formal review checklist. Contact log, meeting-outcome notes, and a review checklist covering annual reviews, progress reports, and filing — the paper trail in one place.

The old way vs. this toolkit
| Task | Paper & memory | With this toolkit |
|---|---|---|
| Finding who gets what | Sticky notes, memory, and digging through plan PDFs | One matrix: student, accommodation, setting, provider, review date |
| Recording daily delivery | Reconstructing the week from memory on Friday | Checkbox P / R / N-A entry per period, the same day it happens |
| Documenting a refusal | Usually never written down at all | Refusals are a built-in column — recorded, not remembered |
| Starting documentation for a new plan | A blank page and your own wording | The 25-item bank, checked off and copied into the matrix |
| Proving parent communication | Scattered emails across your inbox | A dated contact log and meeting-outcome record in the same file |
Questions about this toolkit
How many students can one tracker cover?
The matrix and log pages are designed to be printed or duplicated as many times as your roster needs — one purchase covers your own classroom and caseload use for the school year.
Does it work for both IEP and 504 plans?
Yes. Both plan types live in the same matrix; use the Notes column to record the plan type, and the Expiration / Review Date column to track each plan's annual review.
Why does the log record refusals?
Because a student declining an accommodation is part of the delivery record. If it isn't written down the day it happens, it effectively didn't happen — the P / R / N-A grid makes noting it a one-second checkbox instead of a paragraph.
Is this my district's official form?
No. It is an organizational tool designed to fit common K-12 documentation workflows — check your district's requirements, and keep filing whatever official forms your district requires. This tracker is not legal advice.
