MTSS Tier 2/3 Behavioral Intervention Pack
Built for intervention teams, behavior specialists, and administrators running Tier 2 and Tier 3 supports. Your team writes its own entry rules, review cadence, and exit criteria into the plan form itself, and the one-page student snapshot opens every review meeting with the current data point, the trend, and the next decision date.
What's inside
- Page 1 — Student snapshot. The whole story in 60 seconds: current tier, active interventions, latest data point against goal, trend, next review date, and team contact.
- Page 2 — Intervention plan form. Rule boxes at the top for your team's Tier 2 entry rule, review cadence, and exit criteria — then target behavior, replacement behavior, the step-by-step intervention, frequency, owner, and parent notification.
- Page 3 — Tier 2 weekly tracking sheet. 0–2 scoring per period across the week, undated so any missed week can be backfilled.
- Page 4 — Tier 3 intensification. For when Tier 2 supports haven't produced enough progress: the intensified plan and its review schedule.
- Page 5 — Progress-monitoring graph. Log one row per date, plot one point per date, draw the goal line — the trend is visible to the whole team at a glance.
- Page 6 — Fidelity checklist. Confirms the intervention actually ran as designed — tracking sheet available, data recorded same-day, team reviewed, family updated — before anyone concludes it 'didn't work.'
- Page 7 — Family letter. A plain-language letter explaining the plan, the daily tracking sheet, and how progress will be shared and reviewed.

The old way vs. this toolkit
| Task | Paper & memory | With this toolkit |
|---|---|---|
| Deciding entry & exit | From memory and meeting anecdotes | Your team's own rules, printed on the plan form itself |
| Opening a review meeting | Rebuilding the story from separate notebooks | One-page snapshot: data point, trend, next review date |
| Judging whether it worked | 'We tried it for a while' — with no delivery record | Fidelity checklist first, then the progress graph |
| Keeping families informed | Ad-hoc calls when something goes wrong | Intro letter up front; the daily tracking sheet goes home |
Questions about this toolkit
Who sets the entry rules, review cadence, and exit criteria?
Your team does. The pack deliberately ships with blank rule boxes on the plan form — you write in the thresholds your school already uses, so the form enforces your framework instead of inventing one.
What is the difference between the Tier 2 and Tier 3 pages?
Tier 2 gets the weekly 0–2 tracking sheet for check-in/check-out style supports. Page 4 is the intensification step — a more individualized plan with its own review schedule for students not responding to Tier 2.
How do families see progress?
The page-7 letter explains the plan in plain language, the daily tracking sheet goes home, and the fidelity checklist includes a weekly family-update item so it doesn't get skipped.
Does this replace our district's MTSS framework?
No. It is a documentation layer that fits common K-12 MTSS workflows — check your district's requirements and keep using your district's framework, data system, and official forms where required. This pack is not legal advice.
